    At the recent sharing learning event http://yhhiec.org/themes/pa…..ing-event/ Susanne Cox talked about emerging work in the region around Discharge planning and networking around this. Susannes presentation is at http://yhhiec.org/wp-conten…..rge-v1.pdf. Sussane posed a number of key questions:

    What do you consider to be the three main issues impacting on safe and timely discharge from hospital?
    What changes would you most like to see in the hospital discharge process ?
    What improvements do you think would have most impact for patients and carers?
    How can the Yorkshire & Humber HIEC best support collaborative working to support safe and timely discharge from hospital?

        As we have discovered in our patient reporting research, discharge is certainly of interest (and concern) to patients, and their families. One of the main concerns is often around waiting for medication(s) following discharge. Clearly, this is often a systems issue, but has major implications for the patient, in terms of staying in hospital longer than is neccessary, but also the organisation, in terms the cost of bed blocking.

            Since the Safety In Numbers Sharing Event, we have been scoping out a project to look at improving discharge from hospital, focusing on medicines,communication and teamwork as the key safety issues. We are currently in discussion with key clinicians within the trust to ensure our project proposal fits with the trust agenda and complements existing workstreams etc. We are planning to put in a bid to the Health Foundation SHINE 2012 programme to take forward this work.

            I would be interested in hearing from anyone who is either looking at safety issues across the primary/seconday care interface or is interested in working collaboratively on this issue. I would be particularly interested in hearing from GPs based in and around Bradford.

              In many cases this could start pre-admission (ie. elective surgical patients).

                The SHINE programme looks like a really interesting opportunity!
                From my limited experience (as a non-clinical person) it is effective communication and clear/effective decision making and escalation that have the most impact on the smooth discharge of patients. How can we effectively address this where more than one team is involved?
