About Long Term Conditions for Telehealth
Almost one in three of the population have a long term condition, such as asthma, heart and lung disease, arthritis or diabetes.
Long Term Conditions (LTC) account for 55% of GP appointments, 68% of A&E and outpatient appointments, 77% of inpatient bed days and 69% of the primary and acute care budget in England (“The Big Opportunity”, report from the Regional Director of Public Health, Yorkshire & The Humber, March 2010 )
‘The average cost of someone without a long term condition is around £1,000, which rises to £3,000 for someone with one condition and to £8,000 for people with three or more conditions.’ Health Secretary Andrew Lansley, 14 March 2011 ‘That’s one reason why we need to modernise the health service and the way we care for patients.’ These are just some of the reasons why the YH HIEC Long Term Conditions team are working to deploy telehealth solutions across Yorkshire and Humber.
Focus on …Behaviour Change
THE YH HIEC Long Term Conditions theme have developed a free module in conjunction with experts from across the region, on behaviour change.
This module has been developed with experts from across the Region (incl. Sheffield Hallam University) to provide learners with an increased knowledge of behaviour change theory as well as an understanding of how the key principles and skills can be applied in practice.
The aim of this module is to develop learners understanding of behaviour change techniques. It presents an overview of different approaches and explains the underpinning theory. Concentrating on two approaches, brief advice and brief interventions, this module also aims to develop the core knowledge and skills required for working on a range of behaviour change issues.
On completion of this course learners will:
- Understand the theory on which behaviour change techniques are based and be familiar with a range of approaches
- Be more aware of the negative effects of some common lifestyle behaviours
- Understand the principles and processes of using a person-centred approach when applying brief advice and brief interventions techniques
- Understand the limits of your expertise and be aware of contexts and issues that require more advanced or specialist training
Target audience
This module is targeted at a wide range of people, and is suitable for anyone who may have the opportunity to help individuals manage and support their lifestyle outcomes, for example social care staff, health care staff, as well as carers and volunteers.
