  • What is the local background that gave rise to your initiative or innovation? Who was involved? What is the patient safety problem you were trying to address?
  • What did you do? Please include how you involved appropriate people to develop and support the innovation.
  • What improvement has this made, including early results? How has this addressed your patient safety problem?
  • What has this led to? How can this be applied elsewhere? What is the potential for benefits elsewhere? Tell us how the prize money would help you to deliver those benefits.
